Evaluations
Evaluations are to be completed online post-conference and required for anyone claiming CME/CE credits. Please use the Evaluation & CME/CE Tracking Form that you used during the conference to help you fill out the evaluation.
To be eligible to receive CME/CE, you must complete evaluations for each session you attended and indicate your name, number of credits you are claiming and contact information at the end of the survey. In order to cover GLMA's administrative costs, there is a $30 charge for issuance of CME and CE certificates. If you are a GLMA Major Donor or a GLMA Partner for LGBT Health, your certificate will be complimentary.
